Health insurance card/ patient referral form

The health insurance card is like a credit card used to pay for medical benefits. This proof that you are health insured must be submitted by us to your health insurance company. T he insurance card must be swiped through a card reader machine on your first visit per quarter year. Even if you come just for a prescription or some information, your card is required.

Please be aware that prescriptions debited to your health insurance company may not be issued without proof of your health insurance (i.e. insurance card or referral form). Without this proof you will be given a private prescription (which you must pay for  yourself at the pharmacy).
After submitting proof of your insurance we will issue you a prescription for your health insurance company, with which you can get your money back at the pharmacy.

If you are examined or treated by us, and do not have proof of your insurance, please bring your health insurance card to us as soon as possible to avoid administrative costs and delays - time which we would rather spend on you!

A private bill will have to be drawn up at the end of the quarter year if there is no proof of health insurance!
